Quantum® ELM and ELM2 two-light emergency fixture

What is the hazard?
The circuit board can overheat and cause the fixture to melt, catch fire and ignite nearby items, posing fire and burn hazards to consumers. Lithonia Lighting has received 29 reports of circuit boards overheating and fixtures melting or catching fire, with one incident resulting in more than $100,000 of property damage to an office building. No injuries have been reported.

What are the details of the recall?
This recall involves 1.7 million units of the Quantum® series ELM and ELM2 emergency light fixtures.

What should I (you as a consumer) do if I have a recalled product?
Consumers should contact Lithonia Lighting to receive detailed instructions on how to locate the circuit board and series name and to order a free repair kit, which includes a new circuit board.

What does the product look like?
They are about 12.5-inches wide, 5-inches high and 3.75-inches deep in white or black plastic and have two incandescent lights and a rechargeable lead acid battery. ELM or ELM2 appears on the label inside the fixture’s plastic housing above the battery. The recalled units have circuit boards that are green on both sides and either Rev B or Rev C printed in the top left corner. Emergency lights with circuit boards that are beige on one side and ELM2 LED, ELM2 SD and ELM2 WRS fixtures are not included in the recall.

Electrical distributors, The Home Depot, W.W. Grainger, Inc. stores and other retailers nationwide and online atgrainger.com and amazon.com from March 2010 through February 2014 for between $35 and $55.

What is the product that is being recalled?
Nest Protect: Smoke + CO Alarm

What is the hazard?
Activity near the product during a fire can prevent the alarm from immediately sounding when the Nest Wave feature is enabled.

What are the details of the recall?
This recall includes all Nest Protect Smoke + CO alarms which is about 440,000 units. The alarms are made of black or white plastic and are about 5.25 inches square with rounded corners and about 1.5 inches deep. The word “nest” is on a large button on the face of the alarm. 

What should I (you as a consumer) do if I have a recalled product?
The repair is an automatic electronic update that disables the Nest Wave feature and is delivered automatically to devices connected wirelessly to the Internet and linked to a Nest account. Consumers should take one of the following actions:

Consumers who have not connected their Nest Protect devices to their wireless network and linked them to a Nest account should immediately do so. The devices will automatically receive the update that disables the Nest Wave feature. Customers should confirm that their devices have been updated by going to Nest Sense on their Nest account mobile or web application and ensuring that the button for Nest Wave is off and grayed out. Instructions on how to connect to a network and disable the feature are available at http://support.nest.com/article/Nest-Protect-Safety or by contacting Nest Labs.

Consumers whose Nest Protect devices are connected to their wireless network and linked to a Nest account should immediately confirm the receipt of an automatic repair that disabled the Nest Wave feature by going to Nest Sense on their Nest account mobile or web application and ensuring that the button for Nest Wave is set to “off” and grayed out. No further action is required and consumers can continue to use their devices.

What is the product that is being recalled?
My Sweet Love / My Sweet Baby Cuddle Care Baby Doll

What is the hazard?
The circuit board in the chest of the doll can overheat, causing the surface of the doll to get hot, posing a burn hazard to the consumer.

What are the details of the recall?
About 174,000 are being recalled and the doll is identified by UPC 6-04576-16800-5 and a date code which begins with WM. The date code is printed on the stuffed article label sewn into the bottom of the doll.

What does the product look like?
The My Sweet Love / My Sweet Baby electronic baby doll comes in pink floral clothing and matching knit hat. The 16 inch doll is packaged with a toy medical check-up kit including a stethoscope, feeding spoon, thermometer and syringe. The doll’s electronics cause her to babble when she gets “sick,” her cheeks turn red and she starts coughing. Using the medical kit pieces cause the symptoms to stop. “My Sweet Baby” is printed on the front of the clear plastic and cardboard packaging. 

What should I (you as a consumer) do if I have a recalled product?
Consumers should immediately take the dolls from children, remove the batteries and return the doll to any Walmart store for a full refund.

When were these products sold and for how much?
Walmart stores nationwide from August 2012 through March 2014 for $20.

What is the product that is being recalled and who makes it?
Square D branded, molded case F and K frame circuit breakers by Schneider Electric USA Inc., of Palatine, Ill.

What is the hazard?
The circuit breaker can fail to trip when an overload occurs, posing the risk of fire, burn and electric shock.

What are the details of the recall?
About  28,400 units are under this recall and this recall involves Square D brand models FA, FH, FI and FY one-, two- and three-pole circuit breakers rated 15 to 100 amps, and model KI two- and three-pole circuit breakers rated 110 to 250 amps. The F model breakers were manufactured May 8, 2013 through June 10, 2013 and have date codes 13193 through 13241. 

What does the product look like?
Circuit breakers with the date codes are YYWWD format (example: 13184 = year 2013, week 18, day of the work week 4/ Thursday). The circuit breakers have a yellow label with the words “Square D” or the Square D logo. Model information can be found on the faceplate.

What should I (you as a consumer) do if I have a recalled product?
Consumers should immediately check to see if they have the recalled circuit breakers. Consumers with uninstalled products will receive a replacement. Installed products will be replaced at no charge to the customer with an allowance of up to $300 per site to cover labor costs.

Authorized Schneider Electric distributors nationwide such as Border States Electric Supply, Crescent Electric Supply Co., Dealers Electric Supply Co., Graybar Electric Co., Rexel, W.W. Grainger from May 2013 to June 2013 for between $200 and $9,260.

What is the hazard?

About 4,600 units are being recalled and this recall involves autumn 2013 Gardeners Eden decorative wreaths and Chinese Lantern plant arrangements. The wreaths are made of dried twigs held together with wire and twine. They are decorated with silk leaves, foam berries, papier-mâché pumpkins and gourds in autumn colors or silk leaves and papier-mâché Chinese Lantern fruit.

What does the product look like?
The wreaths are made of dried twigs held together with wire and twine. They are decorated with silk leaves, foam berries, papier-mâché pumpkins and gourds in autumn colors or silk leaves and papier-mâché Chinese Lantern fruit. The plant arrangements are made of twigs with papier-mâché Chinese Lantern fruit held together with wire and twine. LED lights are in the pumpkins, gourds and Chinese Lantern fruit. The LED lights are powered by three AA batteries contained in a black plastic battery box. Style numbers are on the price tags attached to hangtags on the products. 

What should I (you as a consumer) do if I have a recalled product?
Customers should immediately stop using the recalled products, remove the batteries and return the decorations to any TJ Maxx, Marshalls or HomeGoods store for a full refund.

TJ Maxx, Marshalls and HomeGoods stores from about August 2013 to September 2013 for between $25 and $30

What is the product that is being recalled and who makes it?
Air Movers by Packaging Tape, Inc.

What is the hazard?
The air mover/blower's thermal switch can fail and cause the motor to overheat, posing a fire hazard.

What are the details of the recall?
  • 2,800 units are being recalled
  • Models OPS-2500-CF and OPS-2500-CFR are black and model OPS-2200Y-CF is yellow.

What does the product look like?
The air movers’ molded plastic housing measures about 18 inches high by 18 inches deep by 16.5 inches wide and has a 25-foot yellow electrical cord. They have an on/off switch located on the left side of the front of the unit. The model number is available on the sales invoice. The CleanFreak.com logo and phone number are printed on a white label with green lettering on the front of the unit and on a silver or yellow label on the back of the unit. 

What should I (you as a consumer) do if I have a recalled product?
User should stop using the recalled air movers/blowers and contact Packaging Tape to receive a full refund.

What is the hazard?
The heater can overheat and cause the units to melt, catch fire and ignite nearby items, posing a fire and burn hazard to consumers.

What are the details of the recall?
  • 31,000 units are being recalled
  • Duraflame- model DFS-220

What does the product look like?
Units are sold in three colors, black, red and white. The heaters’ flame effect resembles a fireplace and works with or without heat. The heaters measure about 14.5-inches high, 13-inches wide and 8-inches deep. The on-off switch and the high/low heat settings are located on the upper left back side of the unit. The Duraflame logo appears on the front of the unit. The model number can be found on a white label located on the heater’s back panel.

 The garage openers are gray and have a rating of ¾ HPc for the models ending in 1200 and 1 HPc for the models ending in 1500. “Genie” and the model name appear on both sides of the opener. The serial numbers are printed on a label located on the side opposite to the light.

The flashlight can cause the batteries to overheat and melt the flashlight’s plastic casing, posing a burn hazard.

What are the details of the recall?
225,000 flashlights are involved in this recall. The following date codes are included in the recall:  E/J-C, H/F-C, I/F-C, L/F-C, M/W-C, O/F-C, R/F-C, R/J-C,    S/J-C, T/F-C, U/F-C and Y/J-C. The date code is printed on a white sticker inside the flashlight’s tube.  This recall involves Rayovac LED Industrial flashlights (model ILED2AA) with run times of 25 or 50 hours, Rayovac Value Bright LED plastic flashlights (model BRSELED2AA-BA), and Rayovac Value Bright flashlights (model BRSELED2AA-BDGDI). The flashlights use two AA batteries.

What are the details of the recall?
About 42,500 units are under recall and this recall involves Kenmore oscillating fan heaters with model # 127.90914310 (as seen on the the label in the photo above). 

What does the product look like?
The fan heaters are/have... 
  • grey and white 
  • measure about 12" tall by 9" wide
  • two dials at the top, one for fan speed and the other for temperature
  • a red button on the front base to turn the device on and off
  • KENMORE is printed on the front bottom of the fan heater
